Designing the Frontal Hairline | More Than Just a Line

The frontal hairline is the cornerstone of a successful hair transplant. It frames the face, shapes the overall aesthetic, and is the first point of contact with the world’s gaze. But contrary to what many believe, this hairline is not a straight or uniform line. Instead, it is a delicate, irregular composition of subtle peaks and valleys, gradients of density, and carefully orchestrated transitions.

Dr. Juan Couto meticulously recreates this complex pattern using single-hair follicular units in the very front, typically harvested from the finer hairs at the back of the scalp. Just behind, he places two-hair units to create a smooth transition zone, followed by larger three- and four-hair grafts that build internal density and block light reflection. The result is a soft, imperceptible margin that mimics natural hair growth.

To elevate realism even further, Dr. Couto introduces asymmetrical sagittal gradients into his designs. These subtle variations in density and direction — what he refers to as “gradients” — contribute to the illusion of spontaneous, untouched hair. The result is what he calls the “perfect imperfect line”: irregular, asymmetric, and beautifully natural.

Temple Design | Framing the Face from Every Angle

One of the most technically demanding yet visually transformative areas in hair restoration is the temporal zone, or temples. Recession in this area often occurs in tandem with the frontal hairline, and restoring one without the other can lead to an unnatural or unbalanced appearance. Dr. Couto emphasizes the importance of advancing the temples when recession is significant, as they define the lateral frame of the face and provide essential harmony to the forehead and hairline.

Temple reconstruction requires extreme precision. Dr. Couto designs this area using fine grafts placed at very sharp angles, following a declining, organic pattern. It’s an intricate task — a true test of the surgeon’s artistic and technical skill. When done correctly, it transforms a good transplant into an exceptional one.

The Crown (Vertex) | Subtle Complexity, Long-Term Planning

The crown, or vertex, demands a different approach altogether. Its unique spiral pattern requires the careful placement of grafts at precise angles that mirror natural growth. While not as visually prominent as the frontal hairline, the crown plays a critical role in the overall look of full coverage.

There are key considerations when restoring the crown:

  • The growth rate in this area is often slower due to reduced vascularity.
  • Crown recession usually continues over time, even after surgery.
  • A significant number of grafts is required to achieve optimal coverage.
  • Results may be less dense in advanced cases due to donor limitations.
  • Patients under 35 are typically not ideal candidates for crown restoration unless patterns are stable.

For these reasons, Dr. Couto usually prioritizes the frontal 2/3 of the scalp in initial procedures. Crown restoration may be delayed until there is greater clarity on the patient’s hair loss pattern. When appropriate, medical therapy (such as finasteride or minoxidil) is recommended to slow crown recession and preserve existing hair — often with higher efficacy in this zone than in the front.

In cases of advanced alopecia, it is common to approach the crown in two separate sessions, focusing first on strategic coverage, then on density if donor resources allow.
